The side of Diego Simeone came close to the lead in the first half by Alexander Sorloth, who sent a header off-target from the pass by Antoine Griezmann.
But Alvarez, who came just before the hour before Sorloth, scored the winner in the 66th minute with a precise left shot after he was played by Marcos Llorente.
Simeone's sidecar Laliga with 56 points, two free of title rivals Barcelona and Madrid, while they are also confronted with the latter in the first stage of a Champions League Last-16 draw on Tuesday.
Athletic stay celebrated 48 points, six behind Madrid and four free from Villarreal.
