Jack Grealisk was reportedly beaten in the face by a fan of Manchester United in Old Trafford.
The Man City star, 29, was reportedly attacked near the tunnel when he went off the field after the Manchester Derby on Sunday.
A 20-year-old man has since been accused of abuse.
He will appear in July at the Hof van Manchester Magistrates.
Both Man City and Man United still have to give public comments on the incident, although the police are supposed to have immediately launched a probe after reporting the attack yesterday.
A spokesperson for the Greater Manchester police said: “Alfie Holt, 20, of Haven Drive, Droylsden, is accused of abuse.
“He will appear at the Manchester court.
“This indictment relates to an incident that took place yesterday in the Old Trafford Football Club.”
Grealisk started on the couch in front of the city, but stood up for Ilkay Gundogan in the 74th minute of the collision.
The alleged blow comes after sections of the United Crowd 'Phil Foden, your mother sang as ** g' during the boring draw.
City manager Pep Guardiola shot fans for the common song.
After the game he said: “It was a lack of class.
“We are so exposed in football – especially managers, owners and football players.”
He added: “I don't understand the spirit of the people involved in Phil's mother.
“It is a lack of integrity, class and they must be ashamed.”
Foden has also published a message of three words on social media.
“We will continue,” wrote the Man City star alongside five photos of himself from the game.
Fans flowed into the comments to offer the midfielder some support, where one person writes: “Don't pay attention to the Haters Phil, every great person has haters we keep going.”
While another said, “We are with you.”
It comes after the police have promised an increased police in the city at the weekend.
Chief Superintendent Colette Rose, head of special operations at GMP and Gold Commander for the Manchester Derby, said earlier: “We are lucky in Manchester to have clubs with passionate fan bases represented in all football competitions – a Manchester Derby, especially excitement like the city of football.
“There is a police plan for Sunday – fans will note that an increased presence of the police at busier locations and specialized officers are used to ensure that fans stay safe and enjoy the game.
“We want everyone to enjoy the competition – everyone with worries can report them to a police officer or a member of security personnel on the day.”
